NATIVE HEALTH  Community Health Center (NHW Community Health Center) is an Arizona State Department of Health Services Licensed Medical Outpatient Treatment Center.

 

NHW Community Health Center provides primary medical care and is staffed by an M.D. and other professionals. Multiple private insurance plans accepted and a sliding fee scale is available. NHW Community Health Center accepts Health Choice, Care First of Arizona, APIPA, Phoenix Health Plan and American Indian Health Plan AHCCCS plans.

 

Hours of Service

 

Monday-Friday 8:00 a.m.-5:00 p.m.

Call or come in for same day appointments Monday-Friday, 8:00-10:00 a.m. and 1:00-3:00 p.m.

WIC Services - Thursdays 8:00 a.m.-5:00 p.m.

Free medical transportation for appointments at NHW Community Health Center is available on a pre-arranged basis and can be scheduled by calling (602) 279-5351.

 

After Hours:

  • If you are experiencing a medical issue and are an established patient that cannot wait until our regular business hours please call Cosmopolitan Answering Services at (602) 787-3352. You will be directed to an on-call physician or dentist.

  • Free HIV testing (Tuesdays)
  • Prenatal services include prenatal care and delivery (Phoenix Baptist Hospital)
  • Health Start program services 
  • Adult and children's vaccinations (TB, flu shots, tetanus, etc.) - Call for availability
  • Family planning, sexually transmitted diseases (STD) education, treatment and prevention
  • Electrocardiogram
  • Simple laceration repair
  • Rapid strep testing and urinalysis
