NHW Community Health Center accepts APIPA, Mercy Care, Health Choice, Care 1st of Arizona, Phoenix Health Plan and American Indian Health Plan.  NHW Community Health Center also accepts medical RMAP clients. 

 

Family Practice

The NHW Community Health Center physician also provides annual adult physical evaluations and periodic check-ups focused on the prevention and treatment of adult diseases. In addition, sexually transmitted disease prevention, testing and treatment, contraceptive care, free HIV testing, school and sports physicals are also available as well as free pregnancy tests.

Chronic Care Management 

The NHW Community Health Center physician also provides chronic care management to their patients.  Diabetes, heart disease, asthma, and high blood pressure can be managed.

Well Woman Exams

NHW Community Health Center provides thorough well women exams which include clinical breast exams, pelvic exam including a pap smear.  Education; contraception counseling; mammogram referrals; STD education, testing, and prevention; and bone density referrals.  Lab screenings as needed. NHW Community Health Center is a newly selected partner of the Maricopa County Well Women's Health Check Program.  Call for eligibility details.

Healthy Pregnancy – Prenatal Care

NHW Community Health Center provides comprehensive prenatal care. Healthy pregnancies begin with early prenatal care.  Our team is ready to help you learn what you can do to have a healthy pregnancy and delivery. Delivery is at Phoenix Baptist Hospital.

NHW Community Health Center provides support services such as WIC (Women, Infants and Children) and Health Start to assist women and their families. Free transportation is available for medical appointments at NHW Community Health Center. FREE car seat inspections and installation is available through NHW Community Health Center Certified Car Seat Technicians.

Health Start

NHW Community Health Center is devoted to helping expectant mothers, their babies and families remain healthy. Health Start is a supportive program designed for first time pregnant women and teens who would benefit from additional support.  Community Health Workers (CHW) are trained women who provide valuable, home-based guidance for women during  their pregnancies and after birth.

There is no cost to enroll in Health Start. The following services are available:

  • Free pregnancy testing
  • Answer questions about pregnancy and care after the baby is born
  • Information on labor and delivery preparation
  • Home health visit after the baby is born and family checkup
  • Assistance with completing forms and making appointments for services related to AHCCCS and WIC

In order to participate, patients must be pregnant or have a child under age two and must live in Central and/or West Phoenix. Eligible patients must sign a consent form before services can be provided. Pediatrics, Well Child Check Ups, and Immunizations

Pediatric well child visits are important so that the physician can make sure your child is growing appropriately; Office visits can include physical measurements, patient history, sensory screenings, behavioral assessments, and planned procedures (immunizations, screenings and other tests) at the following suggested intervals: *3-5 days after birth; *1 month; *2 months; *4 months; *6 months; *9 months; *12 months; *15 months; *18 months; *24 months; *30 months; *3 years; *4 years and yearly thereafter. 

Diabetes management is available with blood sugar control assistance, glucose machine education, insulin management and diabetic nutrition.  Patient education sessions with our Diabetic Educator are available two Friday mornings per month.

 

FREE TRANSPORTATION FOR MEDICAL APPOINTMENTS AT NHW COMMUNITY HEALTH CENTER is available in Maricopa County on a pre-arranged basis. When you are making your appointment please mention that you would like transportation services.
