NATIVE HEALTH is an Arizona State Department of Health Services Licensed Medical Outpatient Treatment Center.
NATIVE HEALTH provides primary medical care and is staffed by an M.D., RN’s, LPN’s and other professionals.

Scheduled appointments are available Monday-Friday, 7:00 a.m.-6:00 p.m..
You may call or come in for same day appointments. Same day appointments are available Monday-Friday, 7:00a.m.-3:00 p.m.

Free transportation is available in the metropolitan area (Higley to Surprise) on a pre-arranged basis.

*Well women, men and children's check ups
*Immunizations and flu shots
*Family planning, free pregnancy testing, pap smears
*Podiatry Clinic - 1st and 3rd Thursday afternoons of the month. No additional charge
*Speech/language pathology screenings - Tuesday mornings. Available for children 6 months-5 years of age. Part of well child health check. No additional charge.
*Referrals to outside sources
*Acute and chronic disease management, acute illness treatment, minor surgery (toenail removal, wound care, etc.)
*Prenatal care
*Laboratory testing
*School and sports physicals
*Diabetes management
*Free HIV testing

Alternate Resource Representative
Our Alternate Resource Representative is able to provide assistance with health care insurance, AHCCCS applications as well as food stamps (SNAP) and TANF. NATIVE HEALTH utilizes the Health-E-Arizona website to determine eligibility for medical, food/nutrition, and cash assistance programs. If you would like an appointment with the Alternate Resource Representative please call (602) 279-5262 x 3440 or email Letisha at lashley@nachci.com. Please bring the following documents for your appointment: photo ID, social security cards, birth certificates, Tribal enrollment verification (if applicable), proof of income and proof of residency. Appointments are available Monday-Friday, 8:00 a.m.-5:00 p.m.
