Dental Emergencies:
Call for same day appointments, Monday-Friday, 8:00-10:00 a.m. or 1:00-3:00 p.m.
Scheduled Appointments:
Monday-Friday, 8:00 a.m.-4:00 p.m.
Transportation
FREE transportation in the Phoenix metropolitan area (Higley to Surprise) on a pre-arranged basis.
Fee Policy
Effective July 1st, 2009, our Dental Clinic has a new fee policy change. With the exception for the Pascua Yaqui Tribe, all Native American adults who do not have dental insurance coverage or are not covered under a tribal dental program will be charged $20.00 for the initial visit exam to set up the treatment plan.
Insurance
NATIVE HEALTH Dental Department accepts Delta Dental of Arizona, Cigna PPO, TDA, Blue Cross/Blue Shield (PPO), United Health Care, Guardian, Humana, Aetna PPO (out of network), Banner, Ameritas, MetLife, Lincoln, and United Concordia. Other dental insurance is also accepted. Please call for more information.
AHCCCS - Dental
NATIVE HEALTH Dental Department accepts American Indian Health Plan (children, up to age 21), Phoenix Health Plan (children, up to age 21), Care1st Health Plan (children, up to age 21). Please call for more information.
